I live in the state of Colorado and need to terminate my business agreement with my property management company asap. They have breached our agreement and have harrassed my tenants.

No legal form is needed. All you typically need is a letter giving them 30 days notice or what ever amount of notice was agreed to in your agreement.
Depends what your property management contract says. Some contracts have a prescribed beginning and end. (a start and ending date, much like a lease) Others may only require a 30-60 day advance written notice by either party to end the relationship.
If they have breeched your agreement you may have to take formal legal action if you are in the middle of the time period of the property management contract.
